Guidelines- Surveys
Survey data is being collected from as many participants as possible to help determine effects of the XX program. Every Institute should distribute the same survey to all [name of program] participants. Where you feel other data is desirable to obtain, add questions to the survey; do not alter the survey.

When: Surveys should be distributed to teacher-participants the first day of the Institute ('pre' survey) and the last or near last day of the Institute ('post' survey). Follow-up surveys should be mailed during the spring following the Institute; or, if the participants gather together for a follow-up activity in the spring, distribute the surveys to teacher-participants at that time.

Communication: Attached, see examples of a cover letters for the surveys and an example of a reminder note if the surveys are mailed. During the program you may want to remind teacher-participants of the importance of their completing questionnaires in helping you to improve the program and to [name of the program] as a whole.

Procedure:

  1. Distribute the surveys after a brief introduction during which you tell them the purpose of the surveys and remind them that individual responses will be kept confidential.
  2. Answer only procedural questions; do not help them interpret answers. You can say, "Answer the question in any way you interpret it, or leave it blank."
  3. Give the teachers about 20 minutes to read the letter and complete the questionnaire. Give them more time if you have added questions. Guidelines for the protection of research subjects require that filling out questionnaires be voluntary. [Note: the longer the survey, the less likely the respondents will be to provide thoughtful answers and the less likely they will be willing to fill out the questionnaire.]
Mailing the Questionnaire: In the spring, mail the questionnaire including a self-addressed stamped envelope; be sure to attach sufficient postage. Give respondents about three weeks to complete the Questionnaire. Send reminder notes on the due date to those who have not yet responded.

We have asked for the last four digits of your social security number for research purposes only (to help in our statistical analysis). The results will be reported as an aggregate; individual responses may be reported but respondents will remain anonymous.

Please complete the questionnaire and return it to us by [date three weeks after mailing] in the enclosed postage-paid envelope. We have asked for the last four digits of your social security number for research purposes only (to help in our statistical analysis). The results will be reported as an aggregate; individual responses may be reported but respondents will remain anonymous.
