STEP ONE: Each teacher of a team that is accepted to take part in the Multiple Intelligence and Technology Converged course MUST fill out this form. STEP TWO: Fill out this form AFTER your team has been selected to take part in the MI & Tech course. STEP THREE: Read each item, make a choice or type in your answer. STEP FOUR: CLICK the "Finish" button, when you are done. The "Start Over" button will erase all of your answers. It is a good idea to print out your answers on paper, BEFORE you click the "Finish" button. Save the printed pages so that you can see if you learned more about computer technology after you finished the whole project.

This form is used twice. The first time it is used to record what you already know about computer technology, before you start the MI & Tech course. Then, again, this form is used after you have finished the course to see if you learned more about using computer technology.
